MSSW Advising Forms

Below are the advising forms for students to use while in the MSSW program. Students will notice that they are divided up into two main sections: one for on-campus, or face-to-face, students and the other for those enrolled in the Online MSSW program. Furthermore, they are separated by whether a student is Full Time, Extended Study, or Advanced Standing and then by the concentration they are pursuing: Evidence Based Interpersonal Practice (EBIP) or Organizational Leadership (OL).

It is critical that students follow the advising form for their specific program of study exactly as it is written. Failure to do so will very likely delay your graduation date. (There are some courses that are only taught once each year and if you fail to take a course when it’s offered, you will have to wait until the next year before you have the opportunity to take it again.)

All of the forms listed below are in Adobe PDF format.

For those beginning the MSSW Program in the Fall Semester of 2021 (both on-campus and online; regardless of concentration).

For those Extended Study students who started in the MSSW Program in the Fall Semester of 2020 (both on campus and online; regardless of concentration).

Advanced Standing: For all advanced standing students (regardless of when you start/started the program).

Full time: For all full time students who started in the MSSW Program in the Fall Semester of 2020.

Extended Study: For all extended study students who started in the MSSW Program prior to, or in the Fall Semester of 2019.
